What does the notation in the top left mean?

Eighth notes are "swung"; whenever you see two short notes together, don't sing them for equal length as you'd normally do, but make the first longer and the second shorter.

Is this a common notation?

In music in general yes, in hymnals no.

Why are all the verses listed separately rather than stacked?

I guess because the rhythm is slightly different each time, but I also think we could have figured it out. It's not much worse than, say, The First Noel (although a very different musical style).

What is the name of the hymn tune?

"Wet Saints." I get what they were going for, but like...not sure they stuck the landing on that one.
